Steps in opening an online
Term Deposit Account

| 1. Fill in the application form |
| 2. Choose the documents you would like to
submit as Identity, Signature and Address
proof. |
| 3. Choose whether you would like to submit
the application form at the Kotak Bank branch
or you would like an executive to visit you.

| 4. A print version of this form can be submitted
at any Kotak Bank branch. |
| 5. Documents to be submitted along with
the application form (for all applicants)
|
- Photocopy of PAN Card (If you do not
have a PAN, submit form 60).
- Recent photograph of all applicants.
- Documents for Identity, Signature and
Address proof.
- Self signed cheque for placing the
term deposit.* (3rd party cheque would
not be acceptable).
6. All applicants need to come in
person, if you choose to submit this form
to Kotak Bank branch.
* No Cheque will be required if the application
amount has been transferred through NEFT. |

| Terms and Conditions |
| 1. Term Deposit can be opened for a period
of 15 Days to 10 Years (7 Days to 10 Yrs for
Rs.15 Lacs and above & 7 Days to 7 Yrs
for Rs. 1 Crore and above). |
| 2. The Term Deposit is subject to realization
of the cheque. |
| 3. The Term Deposit will be booked with
a value date of clear funds (RBI credit date)
with the Bank on prevailing rate for the day. |
| 4. This application is available for TD
booking by Resident Indian Individuals only.
For NRE / NRO TD, please fill the applicable
form in the NRI Services section. |
| 5. In case the documents submitted along
with the application do not comply with the
KYC guidelines of the bank, the application
is liable to be rejected and the bank will
not be liable to pay any interest on the amount
of deposit. |
